Internal Medicine


   The Department of Internal Medicine was established from the start of Dhulikhel Hospital. This department supports the principles of the hospital and aims to provide quality health care to the patients through competent and qualified medical and nursing staff. The department has ensured that medical knowledge is up to date and also focuses on regular upgrading. The experience and the commitment from those early days have helped the department to grow to a large extent. There is a separate medical ward made up of general wards and a 5 bedded Intensive Care Unit (ICU)/Coronary Care Unit (CCU) service. The department is staffed and equipped to run regular OPD, a 24 hour emergency, and full inpatient services. In additional echocardiography, treadmill test (TMT), comprehensive cardiac assessment, diabetic clinics, Chronic Obstructive Pulmonary Disease (COPD) clinics and ICU/CCU Services are available.

The Department of Internal Medicine has been providing regular OPD, inpatient and ICU/CCU services together with cardiac assessment and investigations (ECG, chocardiography, Ambulatory Blood Pressure (ABP), Holter, TMT). The Department has facilities for bedside echocardiography and Arterial Blood Gas analysis (ABG) service for CCU/ICU patients.

GASTROENTEROLOGY DEPARTMENT  AT DHULIKHEL HOSPITAL:  PRESENT SCENARIO AND FUTURE PLANS
I .  Gastrointestinal services at present
Gastrointestinal complaints are one of the major presenting problems in patients at Dhulikhel hospital. Patients with both acute and chronic complaints attend hospital for want of timely and appropriate  diagnosis and treatment of their problem. There is also a sizable number of patients who suffer from an acute gastrointestinal emergency conditions, such as acute bleeding, foreign body obstruction etc.
Dhulikhel  hospital has been providing gastrointestinal services to patients with gastrointestinal problems since its inception. The department of internal medicine has been providing the spectrum of gastrointestinal services in the medical side .The endoscopic service, among many other tools, forms the main armamentarium in the diagnosis and treatment of many GI disorders.
In the last few years, the range of endoscopic services has been extended to include therapeutic interventional endoscopies .
Endoscopic  services at present:
A. Diagnostic upper GI endoscopy
Biopsy/Histopathology.
Rapid Urease test for H. pylori
Endoscopic FNA from submucosal lesion
B. Diagnostic Lower GI endoscopy
Colonoscopy
Sigmoidoscopy
C. Therapeutic endoscopic services that can be done at present
1. For GI bleeding
Endoscopic variceal band ligation.
Endoscopic injection therapy for acute bleed
Endoscopic clipping for acute bleed
Bipolar electrocoagulation for bleeding lesion
PEG tube insertion
2. For oesophageal stricture
Oesophageal bougienage (Wire-guided))
3. Removal of foreign body in oesophagus
4. Endoscopic snare polypectomy
5. ERCP- sphincterotomy/CBD stone extraction/stent placement/nasobiliary tube insertion
I . Future  Plans
A. Separate Gastrointestinal Department:
The hospital will have a separate gastro-intestinal block  with well-designed modern endoscopy suite. The design of the unit has been supported and approved by the GASTROENTEROLOGY FOUNDATION e.V. GERMANY The construction of the building is underway and expected to be complete by the end of 2012.
The name of this new GI block has been suggested as “ Nepal Endoscopic Training Center
(Nepal ETC).”
B. GI Endoscopy training and research institute:  apart from providing a day to day gastrointestinal services to the patients, the gastrointestinal department  will also plan and extend its services to teaching endoscopic skills to other physicians & surgeons from all-over the country. In the future, the department will also contemplate to starting a fellowship/superspecialization training in gastroenterology. The department will also focus to  conducting  researches on gastrointestinal conditions and publish papers on national and international journals.
C. Human resource recruitment
The gastroenterology department will have more trained physicians and GI assistants in the future. The regular hands-on training of qualified physicians & assistants will need to be continued.
D. Scienticfic Gastro conference: The department is going to organize an international GI conference in October 2012, where physicians from across the country and abroad will be presenting research papers along with discussions and talks on many aspects of gastrointestinal disorders. There will also be paper presentations from our hospital.
E. Upgrading of gastrointestinal investigative tools:
1. Ultrasound/ Endosonogram: provision for a separate ultrasound machine for routine gastrointestinal patients is necessary. Simple US –guided procedures will be done in the department. We will also plan to have an endoscopic sonogram machine in the near future which will help and add diagnostic accuracy in many GIT tumors and also add to performing some therapeutic endoluminal procedures.
2. Enteroscopy / double balloon endoscopy/capsule endoscopy:  as being envisioned as a referral training institute in the country, the gastrointestinal department of Dhulikhel hospital will consider  in employing this investigative tool as next step in its development course.
3. Adding other tests: such as, Urea breadth test for HP, oesophageal PH monitoring /manometry.

Postgraduate MD/MS degree program for Academic Session: February 2012
On the basis of Entrance Examination held on 11 February 2012 and subsequent interview the following candidates have been selected according to the selection criteria published on 31 January 2012 for admission in the PG program in the institutions under Kathmandu University as per the attached list (pages 6). The candidates must take admission by 29 February 2012.
All the selected candidates are informed to contact their respective training sites for their admission process. The admission form with necessary documents (SLC- mark sheet and character certificate, I.Sc./10+2- transcript and character certificate, MBBS - degree, Internship completion certificate, NMC registration certificate, Citizenship certificate and Bank voucher) must be forwarded by the concerned training site by 1 March, 2012.
For admission each candidate has to submit the following documents at the concern training site by 29 February 2012:
1.Duly filled forms provided by the University along with necessary documents.
2.Bank voucher of NRs. 3,40,000/- for Clinical Sciences (including Pathology but excluding Forensic Medicine) and NRs. 2,13,000/- for Basic Sciences (including Forensic Medicine but excluding Pathology) at Nepal Investment Bank Ltd., in the Saving Account KUSMS, No. 005 8261140 asuniversity fee.
3.The payment of fees will be strictly according to the University rules as follows:
